Waiver The Burg Cage

1. In consideration for being permitted to participate in baseball training with The Burg Cage LLC and Kyle Chaves Baseball Training LLC, I, as the parent/guardian, hereby voluntarily RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE any representative of  The Burg Cage LLC for any and all actions or causes of action, including negligence, for injury as a result of my child/ward participating in baseball training. I, further, on behalf of the player/participator, assume all risk and injury either directly or indirectly, related to the player participating in baseball training with The Burg Cage, LLC or Kyle Chaves Baseball Training, LLC.

2. I understand that I, parent/guardian, am responsible for paying the cost of treatment for any injuries sustained by my player while participating in any baseball training with The Burg Cage, LLC or Kyle Chaves Baseball Training, LLC.
